Lipoprotein 지단백, Cholesterol 콜레스테롤

A lipoprotein is a biochemical assembly whose primary function is to transport hydrophobic lipid (also known as fat) molecules in water, as in blood plasma or other extracellular fluids. They consist of a triglyceride and cholesterol center, surrounded by a phospholipid outer shell, with the hydrophilic portions oriented outward toward the surrounding water and lipophilic portions oriented inward toward the lipid center. A special kind of protein, called apolipoprotein, is embedded in the outer shell, both stabilising the complex and giving it a functional identity that determines its role.

 

<Classification by Density>

• Chylomicrons carry triglycerides (fat) from the intestines to the liver, to skeletal muscle, and to adipose tissue.
• Very-low-density lipoproteins (VLDL) carry (newly synthesised) triglycerides from the liver to adipose tissue.
• Intermediate-density lipoproteins (IDL) are intermediate between VLDL and LDL. They are not usually detectable in the blood when fasting.
• Low-density lipoproteins (LDL) carry 3,000 to 6,000 fat molecules (phospholipids, cholesterol, triglycerides, etc.) around the body. LDL particles are sometimes referred to as "bad" lipoprotein because concentrations, dose related, correlate with atherosclerosis progression.

+) Lipoprotein little A - Lp(a) : LDL과 비슷한 구조, is comprised of an LDL-like particle with a second protein, called apolipoprotein(a), or apo(a), coiled around it. Recent studies indicate that people born with elevated LP(a) may have a two-fold to four-fold increased risk of heart attacks and other serious events compared to people with low Lp(a) levels.

 

<콜레스테롤>

전신에 존재하는 모든 세포의 막을 형성하는 지질의 한 종류로 생명에 필수적인 물질입니다. 콜레스테롤은 체내의 막 표면에 있으면서 막을 보호하고, 혈관벽이 찢어지는 것을 예방하며 적혈구의 수명을 오래 보전시킵니다.

 

따라서 콜레스테롤이 부족한 경우 적혈구의 수명이 짧아져 빈혈이 생기기 쉽고, 출혈성 질환의 위험도 증가될 수 있습니다. 반대로 혈중 콜레스테롤에 높은 경우 동맥벽에 침전물을 형성하여 동맥경화증을 일으킬 수 있으므로 적정량의 콜레스테롤을 유지하는 것이 중요합니다. 

 

체내 콜레스테롤은 지단백과 함께 혈액내를 순환하는 HDL 콜레스테롤과 LDL 콜레스테롤을 포함하는 총 콜레스테롤을 측정합니다. 이 검사는 질환을 진단하는 데에 사용되지 않고, 심혈관질환의 위험도를 예측하고 콜레스테롤 수치를 조절 중인 사람의 추적검사로 사용됩니다.

 

모든 성인은 적어도 5년에 한 번 씩 검사하는 것이 좋습니다.

총콜레스테롤 200mg/dL 이하
LDL콜레스테롤 130mg/dL 이하
HDL콜레스테롤 60mg/dL 이상
중성지방 150mg/dL 이하

[이상지질혈증의 진단] 적어도 2회 이상의 측정에서 이 중 하나라도 이상이 발견되면 이상지질혈증이라고 불릴 수 있다.
이 기준 수치는 심장마비나 뇌졸중의 위험요인이 없는 사람들의 목표치를 의미하는 것이며, 심장질환이 있거나 위험요인이 있는 사람들의 정상범위는 달라질 수 있다.

[이상지질혈증의 치료]
운동 및 식이요법 등의 생활습관 교정과 더불어 약물 치료가 이상지질 치료의 핵심이다. 스타틴(statin), 에제티미브(ezetimibe) 및 피브레이트(fibrate) 계열의 약물이 가장 흔히 쓰이며 최근에는 PCSK9 억제제 등이 이용되고 있다. 이 약물의 복용 기간에는 금주가 요구된다. 이상지질혈증의 치료은 고지혈증의 치료 지침과 같다.

약리작용

• 피브레이트(fibrates) 제제는 PPAR-alpha* 효능제로 작용하여 아포지단백 C-III(지방분해 효소의 작용을 방해)의 기능을 저하시키고, 아포지단백 A-I(고밀도지단백의 주요 구성 물질)의 기능을 증가시키는 등 지질단백질을 분해하는 효소의 작용을 돕는다. 그 결과, 혈중 중성지방 수치를 낮추고 VLDL 콜레스테롤의 분해를 증가시켜 LDL 콜레스테롤의 수치를 낮춘다.
 
• 오메가-3 지방산(omega-3 acid ethyl esters)은 중성지방 합성과 관련된 효소를 억제하고 지질단백 분해를 돕는 효소의 활성을 증가시켜 더 많은 양의 중성지방을 제거한다.
 
* PPAR-alpha(peroxisome proliferator-activated receptor-alpha): 핵전사인자로서 간에서 지질 대사과정에 관여하는 주요 조절제 역할을 한다.

 

효능∙효과

• 피브레이트 제제는 콜레스테롤이나 중성지방이 높아져 있는 이상지질혈증을 치료하는데 사용된다.
 
• 오메가-3 지방산은 중성지방이 상승된 이상지질혈증 환자에서 중성지방 수치를 감소시키기 위한 식이요법 보조제로 사용된다.
