Wood's lamp test : 진균이 생성한 물질 관찰

- 머리백선 tinea capitis : 황녹색

- 어루러기 tinea versicolor : 황금색

 

직접도말 검사 KOH 도말

- 칸디다증 candidiasis : 균사 hyphae나 거짓균사 pseudohyphae 존재

 

Tzanck 도말 검사

단순 포진, 수두, 대상포진 - multinucleated giant cell 관찰

 

# 습진성 피부 질환

접촉피부염 contact dermatitis (자극접촉피부염-주부습진, 알레르기접촉피부염)

아토피피부염 atopic dermatitis (유아기~2세 : 뺨, 이마, 머리, 사지 폄쪽 습진 / 2세 이상 : 접히는 부위)

지루피부염 seborrheic dermatitis ~ Malassezia Genus 관여, 두피 등 피지 분비 왕성한 부위

신경피부염 neurodermatitis

동전습진 nummular eczema : 팔다리 폄쪽, 심한 가려움.

건성습진 xerotic eczema : 피부 건조. 열창 fissure, 비늘 scale

 

# 홍반

결절홍반 erythema nodosum : 양측 다리 폄쪽, 사슬알균 감염 

경화홍반 erythema induratum : 중년 여성 하지 굽힘쪽, 결핵

다형홍반 erythema multiforme : HSV 감염, Target 모양 병변 대칭적

 

# 약물 관련

발진약물발진 exanthematous drug eruption : 약물발진 중 m/c, 홍반성 반점 구진 전신에 대칭적.

두드러기 약물발진

고정약물발진 fixed drug eruption

Stevens-Johnson Syndrome SJS/ 독성표피괴사용해 TEN

약물과민반응증후군 DRESS drug rash with eosinophilia and systemic symptom = DIHS drug induced hypersensitivity syndrome

 

# 구진인설성 질환

건선 Psoriasis : Koebner 현상(외상부위 유발), 진단-punch biopsy, Auspitz 징후(인설 제거하면 점상출혈)

장미색잔비늘증(장미색 비강진) Pityriasis rosea : 몸통 및 사지 구진인설성 발진. 자연소실-경과관찰

편평태선 Lichen planus : 심한 가려움 동반(건선과 차이), 굽힘쪽 대칭적, Koebner 현상

 

# 옴 Scabies

permethrin, ivermectin

 

# 얕은 피부진균증

백선 Tinea - 피부사상균 (Trichophyton, Microsporum, Epidermophyton), 국소 항진균제 (+ 경구 항진균제 - 머리백선, 손발톱진균증)

어루러기, 전풍 Tinea Versicolor : Malassezia 효모균, 땀이 많은 젊은 사람, 광범위 항진균제 연고 + 경구 항진균제(Itraconazole)

 

# 세균감염

고름딱지증, 농가진 impetigo : 여름철 소아 - 수포농가진(S. aureus) / 접촉전염농가진(S. aureus, Group A beta-hemolytic streptococcus) - 항생제 연고 국소 도포

얕은 농포모낭염 Superficial pustular folliculitis

포도알균 열상 피부 증후군 SSSS Staphylococcal scalded skin syndrome

피부결핵 tuberculosis of the skin

오목각질용해증 pitted keratolysis

 

# 바이러스 감염

물사마귀, 전염물렁종 Molluscum contagiosum - umbilication 구진, MCV(molluscum contagiosum 바이러스)

대상포진 Herpes Zoster - Tzanck 검사에서 다핵거대세포

 

# 피부 종양

광선각화증 actinic keratosis - 편평세포암 이행 가능 암전구증

보웬병 Bowen's disease - 표피내 편평세포암, 암전구증

기저세포암 Basal cell carcinoma 

편평세포암 Squamous cell carcinoma
